IVF After IUI Failure: Understanding When It’s Time to Take the Next Step and Move to IVF
By Susan Davies, M.D.
How Many IUIs Are Typically Recommended?
Intrauterine insemination (IUI) is often one of the first fertility treatments patients pursue. It is less invasive than IVF and can be effective for certain diagnoses. However, when multiple IUIs do not result in pregnancy, it’s natural to wonder what comes next.
While each case is different, most fertility specialists recommend three to four well-timed IUIs before reassessing. After this point, success rates tend to plateau, particularly for patients over age 35 or those with underlying fertility challenges.
Why IVF May Offer Higher Success
IVF provides several advantages over IUI:
- Controlled fertilization in the laboratory
- Close monitoring of embryo development
- Selection of embryos with the strongest potential
- Optional genetic testing in appropriate cases
IVF allows us to better understand where challenges may be occurring — whether during fertilization, early development, or implantation.
Emotional Considerations - When to Move to IVF
Deciding to move from IUI to IVF can bring mixed emotions. Many patients feel a sense of grief that a simpler approach did not work, alongside hope that IVF offers new possibilities. At Davies IVF, we prioritize education and emotional readiness — not pressure.
Making the Decision
The decision to move forward with IVF should be guided by:
- Age and ovarian reserve
- Diagnosis and treatment history
- Emotional readiness
- Personal goals and timeline
